This information comes from none other than our bignaturals toting friend, Withers. A throw-away line reveals that 120 days have passed from the time the Nautiloid crashes to when you defeat the Netherbrain. Of course, like with many things in the game, it’s triggered by specific events.
As spotted byTheImageworkson Reddit, should you let Karlach die at the end of the game, Withers will say, “In but a dozen tendays a lifetime was lived, More than mortal years. Mortal centuries were hers.”
“From the moment Karlach left Avernus the same day game events start to the day she dies on the docks after the Netherbrain fight is “a dozen tendays” i.e. 120 days. That’s how long the game takes place.”
Four months sounds a bit short, considering how much time we all spent in the Underdark or exploring the upper levels of the great city. However, most players think it canonically makes sense.
